Financial and Health Ed Foundation, dba “Reintegration Services”

Founded in June 2022, our organization is dedicated to strengthening the San Benito County and Monterey County by providing compassionate support and practical solutions for those facing life’s toughest challenges. We serve veterans, unhoused individuals, justice-involved, and other vulnerable populations who too often fall through the cracks of fragmented systems of care.

As of October 2025, we’re proud to share two major milestones:
✅ We’ve established a new office space to expand our in-person services.
✅ We’re now an approved Medi-Cal Provider with California’s Department of Health Care Services (DHCS)—a critical step toward becoming an approved Medi-Cal Provider with CCAH offering even more impactful programs through the Central California Alliance for Health.

(Starting in April 2026 we will be sub-contracted to provide CalAIM CHW Benefit Services using the platform “Pear Suite”, which is contracted and credentialed with CCAH!)

(Starting in April 2026 we are also seeking a partnership with The United Way of Monterey County to provide Financial Coaching and CHW Services at their Community Impact Center facility at 232 Monterey Street, Salinas, CA)

Our nonprofit was founded by L. Mitchell, RN, and D. Bouie in June 2022, with the aim of helping people in our community who are underserved and in need of help, from the unhoused, the previously incarcerated, to all others who are at-risk!

We have been a client of CSUMB’s IIED Program since 2023. In October 2025 our nonprofit became an approved Medi-Cal Provider with the (DHCS) Department of Health Care Services! Using the Pear Suite platform, starting in April 2026 we will provide CHW services to our Medi-Cal Members, Private Insurance Members, and others seeking to better themselves and their situations.

As always, we will be seeking any help in acquiring building/office space for a future Shelter and an (LTORR) Transitional Reentry Program for the Justice Involved, and others who are substance abusers, unhoused, or on probation, that local and State Law Enforcement contractually sends to us to attend our future, various self-help programs!

We envision a future where everyone has access to the resources, support, and guidance they need to build a positive and fulfilling life.

How We Help

Starting in April 2026 our volunteer Executive Director, and volunteer Credentialed CHW, Daryl Bouie will be providing CHW Services in Monterey and San Benito Counties. He and future CHW Interns will meet people where they are, by 2-1-1 Referrals and the United Way of Monterey’s Smart Referral Network system — out in the community — and in-office — offering the tools and support they need to achieve health and financial stability and independence.
We provide:

  • We help people apply for Medi-Cal and with renewal of Medi-Cal benefits

  • Utility assistance by helping members apply for CARES

  • Life-skills training, including financial literacy coaching and health education

  • Donation and resource assistance for our unhoused and underserved members

  • Guidance and empowerment for individuals transitioning from incarceration

With our new location and growing partnerships, we’re excited to expand our services and continue building pathways to hope, healing, and self-sufficiency.
